
Patient Responsibilities
New and Current Patients
All patients will be asked to sign the required authorization forms when coming to our clinics, ER, and hospital. You are required to bring your current insurance cards & photo id (for protection of your health information) to each appointment. It is very helpful for the providers if you bring a copy of your current medications to each appointment so they can be updated accordingly. If you are a new patient it is very beneficial to have your previous provider send your records to our clinic.
Insurance & Billing
FCMH and our medical clinics participate with many insurance programs including but not limited to BC & BS of Nebraska, Midlands Choice, United Health Care, Aetna, Medicare, & Medicaid. Our office files claims to most insurance companies but ultimately you are responsible for settling your account. If you have any questions, please contact our Finance/Billing department at 308-425-6221.
At FMCH, we offer convenient, secure, online payment options (Pay My Bill), we accept cash, Visa, Mastercard, and Discover cards. Fees for service or insurance co-payments are required at the time of service.
Tobacco Free Campus
We are a Tobacco Free Campus. Medication is available to patients to curb their craving for tobacco. We also offer information on Tobacco Cessation.
Visiting Policies and Hours
While normally at FCMH we would have no set visiting hours because we recognize that a loved ones' presence help in the healing process. We do ask that all visitors, in the face of the COVID-19 Pandemic, we are limiting visitors to only 2 immediate family members per day for those not in isolation and only 1 visitor for those in isolation until the pandemic has been controlled.
