

The FCMH Patient Portal is a secure web-based system that will allow you to receive communication from our office and access portions of your medical records.
Once you have signed up for the Patient Portal, these services are available online anytime through your computer allowing you to communicate with our office at your convenience.

How do I sign up for the FCMH Patient Portal?

CLICK HERE to go to the FCMH Patient Portal and create your free account today.
Or you can show the front office staff at the Pool Medical Clinic or FCMH Outpatient Services your photo ID and give them your personal email address and they will sign you up for the portal and receive an email with a link to the portal, PIN and next steps.

When you click the link, it will take you to the portal and you are able to log in for the first time and enter your assigned PIN. You will have the opportunity to create your own password. Once logged in, you can begin to access your patient information.

Frequently Asked Questions
Q: Why can't I register for the FCMH portal over the phone?
A: For security reasons, we need to register you in person in the office.
Q: What if I can't remember my password?
A: Go to the portal site and click on the "forgot password" link and follow the directions.
Note: it may take 10-15 minutes for your new password to become active.
Q: Why can't I see all of the information my record?
A: The portal does not allow all portions of your medical record to be visible. Test, visits, vaccines, etc. that were done prior to initiation of our portal, September 2016, will not be visible.
